3/17/2023 EnchantaVaughn Approved 1.Behavior: Upon arrival, Client appeared to be in a content mood evident by her facial expressions such as a smile on the face. QMHP-C engaged Client in a discussion to determine behaviors since the last session. Client reports she continues to have anger issues and she is going to as long as people treat her any type of way. 2. Behavior: During the second portion of the session, client presented issues with being able to implement coping skills when she is upset. Mother reports she has the biggest challenges with client when it comes to her anger and understanding boundaries. 1.Intervention: QMHP-C reflected with Client on her choices, reviewing both good and bad experiences. QMHP-C processed with Client, "what would you do different tomorrowâ. QMHP-C encouraged Client to have a good evening. QMHP-C encouraged Client that tomorrow is a new day. QMHP-C praised Client for utilizing acquired skills and being able to get back on track without episode. QMHP-C supported Client with displaying positive social interactions with peers and authority. QMHP-C reinforced appropriate coping skills in social setting. QMHP-C processed Client positive responses and challenged Client to maintain appropriate behaviors throughout the entire week. QMHP-C encouraged Client to resist negative behaviors in efforts to refrain from requiring to be redirected by her mom so many times throughout the day. 2. Intervention: QMHP-C observed Client complying with home rules and expectations. QMHP-C utilized empathetic listening skills as Client expressed her understanding of stressful situation. QMHP-C provided age-appropriate understanding of the situation and why she is there to help resolve any issues she has, including her having difficulty completing chores or assigned work. QMHP-C informed Client that refusing to do her work is not acceptable and she should take small personal breaks involving deep breathing and stretching in her seat without initiating conflict or making loud noises. QMHP-C encouraged Client to be mindful of others personal space and knowing when to set boundaries. QMHP-C encouraged Client to refrain from displaying negative behaviors towards her mom, to talk calmly and express herself without aggression. QMHP-C helped Client to see the positives and to take advantage of receiving additional help at home. 1. Response: Client developed understanding of academic expectations, she reported increased satisfaction and function. Client responded well to feedback, interacting well with others. Client was able to speak calmly and demonstrate her improved communication skills while engaging in self-reflection. Client implemented her plan for restoring relationships, and this was reviewed. Client engaged in self-reflection, discussing good and bad parts of her day. Client said, "thank you". Client chose to get back on task, as observed by her listening and following directions. Client appeared to be learning from her mistakes and correcting them throughout the day. Client was acceptive of encouragement and decided to listen to her mom and do the right thing. Response: Client was seen following session rules and displaying good expectations in the home, without requiring redirection or needing a break. Client completed the activity without any interruptions. Client was praised for her cooperation. Client realized her mom was trying to help her by pushing her to try first and then get more help from counseling. Client nodded in agreement and promised to do her work so that she can earn credit and get a reward at the end of the week. Client was respectful and careful not to invade in authority business, displaying age appropriate conduct throughout the session. Client focused on things she is good at and loves to do at home in order to stay motivated in the therapeutic setting and make something out of herself. Client dismissed from session in a calm manner, as observed.
